Blepharoplasty, commonly known as eyelid aesthetics, is a surgical procedure aimed at addressing the excess skin, fat, and muscle around the upper and lower eyelids, primarily caused by aging. Over time, the skin loses its elasticity, and the muscles weaken, leading to drooping eyelids and puffiness. These changes can not only result in a tired and aged appearance but may also impact vision by narrowing the field of sight, particularly in cases of severe sagging on the upper eyelid.

The primary goal of blepharoplasty is to rejuvenate the eye area, giving the face a more youthful and refreshed appearance, while also addressing any functional issues such as impaired vision. This procedure is typically recommended for patients over the age of 35, although younger patients with significant hereditary puffiness or drooping may also benefit from it.

Upper Eyelid Surgery

The most common issue with the upper eyelids is the excess skin, which can cause a heavy, drooping appearance. In some cases, patients may also experience fat accumulation, particularly in the inner corner of the eyelid, leading to a bulging effect. During upper eyelid surgery, excess skin is carefully removed, and if necessary, the excess fat that causes puffiness is also taken out. The procedure is always performed using an open surgical technique, with the incision made along the natural crease of the eyelid, making the scar virtually invisible once fully healed.

Lower Eyelid Surgery

Lower eyelid blepharoplasty is performed to address puffiness, sagging, and wrinkles below the eyes. If the issue is primarily due to fat pockets, the surgery can be performed internally, by making an incision from the inside of the lower eyelid (transconjunctival approach), which leaves no external scar. However, if excess skin is present, an incision is made just below the lash line, ensuring minimal visible scarring. After excess skin and fat are removed or repositioned, the result is a smoother, tighter lower eyelid with a more youthful contour.

Recovery and Post-Surgery Care

Since the skin of the eyelids is the thinnest on the body, it heals remarkably well, making scarring less noticeable compared to other areas. In fact, many patients find that they have little to no visible scars after healing. Eyelid surgery is typically performed under local anesthesia, and the procedure takes about 1 to 1.5 hours, depending on whether both upper and lower eyelids are being treated. Most patients can be discharged shortly after the procedure, making it a convenient outpatient surgery.

Post-operative recovery is relatively quick. Most patients can return to their normal activities within 3 to 5 days. Mild swelling and bruising around the eyes are expected, but these usually subside within a week. Cold compresses and elevation of the head can help reduce swelling. While initial results are visible as soon as the swelling decreases, the final outcome of the surgery becomes apparent within a few weeks to a few months, as the tissues continue to heal and settle into their new contours.

Follow-up appointments are essential to monitor the healing process and ensure that the results align with the patient’s expectations. With proper care, the results of blepharoplasty can last for many years, offering long-term improvements in both appearance and function.
